Chest pain, trouble breathing, signs of a stroke or a serious injury: call 911.

Then text us, and we'll help with what comes next.

    After hours

Members text or call (850) 555-0143 any time. Evenings, weekends and holidays, the clinician on call answers urgent messages within 30 minutes. Routine questions wait for the next weekday.

    What membership doesn't cover

Hospital care, emergency rooms, specialists and major imaging. Keep health insurance for those. Many members pair membership with a high-deductible or catastrophic plan.

    If you have Medicare

In this demo, both clinicians have opted out of Medicare. You'd sign a short private contract, Medicare won't pay for our services, and you keep Medicare for everything else. People on Medicare can't contribute to an HSA.

    The agreement, in plain words

    Not insurance
Membership pays for primary care from Saltmeadow only. It is not health insurance and does not cover emergency rooms, hospital stays, specialists or imaging.
    Minimum
Stay at least 3 months. After that, it is month to month.
    Leaving
Give 30 days notice by text, email or in person. No cancellation fee.
    Coming back
Rejoin within 12 months of leaving and there is a $99 re-enrollment fee.
    Price changes
Your price is locked for 12 months. Any change after that comes with 60 days notice.
    Enrollment fee
$99 per person, never more than $199 for a household.

    Membership for your team

Employees and their families get the same age-based prices, billed to you once a month. Groups of five or more get enrollment fees waived and one person here who handles your account.

Tell us about your group and we'll send a quote built from your actual headcount.

    Questions people ask

Do I still need health insurance?

Yes. Membership covers primary care at Saltmeadow. Keep coverage for emergencies, hospital stays, specialists and major imaging. A high-deductible or catastrophic plan is a common pairing.

    Do you bill my insurance?

No. Nothing in the membership is billed to insurance. Labs and medications are paid directly at the prices we list.

Can I keep contributing to my HSA?

IRS Notice 2026-5 sets a fee limit for direct primary care: $150 a month for one person, $300 for more than one. Every Saltmeadow price is under it, and the household calculator checks yours. The fee is only one of the conditions, so ask a tax advisor before counting on it.

How fast can I be seen?

Sick visits are same day or next weekday. A first visit for a new member is usually within a week; the current wait for each clinician is shown at the top of this page.

What happens when I need a specialist?

We pick the right one with you, send your records, and often talk to them directly. The specialist bills your insurance.

    Do you prescribe controlled substances?

We don't keep them in the clinic. When one is the right treatment, we send the prescription to your pharmacy.

    What ages do you see?

Everyone, from newborns on. Children join alongside an enrolled adult.
